Dental/Medical Insurance

Your insurance is a contract between your employer or your University and an insurance company. Benefits received are based on the terms of the contract negotiated between your employer and the insurance company.

Our office will do everything possible to help you understand and make the most of your dental insurance benefits. Before any planned procedure, we will provide you with a pre-determination form to submit to your insurance company; this will allow you to know in advance what portion of the fees will be covered. After each treatment is complete, you will be given a claim form to recover the appropriated amount from your insurance company. Please remember that you are ultimately responsible for all expenses incurred. We urge you to read your insurance policy so that you are fully aware of coverage and any limitations of the benefits provided.
